Make sure all ramp PHDU cars are added on creation

When amending the PHDU, I add a card to indicate that it has been patched. Make sure to add that same card with F when creating the PHDU. Otherwise, when amended the PHDU can grow to take up an entire new FITS block, so amending has to take the time to rewrite the entire file.
