[INSTRM-1207] add annotation_id and timestamp to sps_annotation table

sps_annotation table does exist but isn't currently very useful.
We would like the ability to add more than one annotation per file, having the date as well.
So basically two new columns:

annotation_id as primary key (int4 not null)
created_at (timestamp)
