Update grandfibermap following the recent discussions and requests

Following your suggestions and requests, I added </p> <ul> <li>blank and engineering fibers</li> <li>fiberid</li> <li>cobra module ID</li> <li>cobra board ID, and</li> <li>cobra number in one board</li> </ul> <p>I changed header names: <tt>mod</tt> is now for cobra board ID, not subgroup for fiber MTP ferrule. I hope this change sounds reasonable.<br/> I also sent an email to Jim for his review.</p> <p>Craig is looking at it, but needs to do more checks.
