Update grandfibermap with all Cable A-B, B-C, B-S interface mappings

The grandfibermap data file gives the cobra-to-slit fiber mappings, and uses a single "fiber id" which includes the field, spectrograph, an MTP number, and the cobra-in-module number.

That ID may or may not be the right one, given what we now know about the A-B-C MTP mappings.

In any case, we need to add all the MTP mapping for the end-to-end system.

Also, we are adding SuNSS, and it might be worth adding the SuNSS fiber ids to grandfibermap.
