update request for new github repository procedure with less interaction

we may reduce some interactions in this procedure.

need to be taken considered are:

• we place official review required for any commit to pfs_www_pipe:master, but could be done internally within the project office (or admin group)
• need to secure access to new github repository from requestee, it is better to ask requestee closing/resolving a ticket when they confirms it is working.
